Two former Celtics point guards are on the move together.

The Cleveland Cavaliers have reached an agreement to trade Delonte West and Sebastian Telfair to the Minnesota Timberwolves for guard Ramon Sessions and forward-center Ryan Hollins, according to Yahoo.

West played for the C’s for three seasons from 2004-07, before being traded to Seattle in the Ray Allen deal. Telfair’s only season in Boston was in 2006-07, and he was moved to the Timberwolves in the Kevin Garnett trade.

The T-Wolves are expected to waive West, who is only guaranteed to make $500,000 of his $4.7 million salary next season. Cleveland will also receive a second-round pick in the deal.
